A national passport, valid for at least three months after the expiration date of the requested visa and having at least one blank visa designated page
One Visa Application form (it has to be printed out from the Ministry of Foreign Affairs site: visa.mfa.gov.ua , you will need Acrobat Reader to view or print these forms) per each passport holder - answered completely, dated and personally signed by the applicant aged 16 or over or by a parent or a legal guardian of the applicant under 16.
One photo - recent, passport-sized, resembling applicant's travel document photo.
Invitation letter (approved by the State Migration Service of Ukraine) or tourist voucher (issued by the Ukrainian tourist company/hotel authorized to provide tourist services for the foreigners)
Copies of the data-page(s) of Applicant’s Passport and the Qatari Residence Permit card (for Non-Qatari citizens).
Company letter (position and the salary must be mentioned)
Bank account statement for the last 3 months.
